New Patients

At Gateway Pediatric Dentistry, your children’s smiles are important to us! We keep kids happy and healthy by focusing on providing exceptional service to promote bigger, brighter smiles. Our goal is to provide a pleasant and positive experience for both you and your children while focusing on your specific needs.

We understand how intimidating visiting a dentist for the first time can be for children. For this reason, we strive to have a welcoming and comfortable office for you and your children.

Prior to your children’s first visit, feel free to ask our friendly staff any questions you have – we’re here to help alleviate any concerns you have about coming to see the dentist. Our new patient forms are available for you to download and fill out before you come see us.


Please note that requesting an appointment time on this form is not a guarantee of booking. One of our staff members will contact you within one business day to book. 
Once your appointment has been booked, the time slot booked has been reserved for you. We will contact you prior to your appointment to confirm the date and time. In consideration of our staff and other patients waiting for appointments, we request at least two (2) business days’ notice of all cancellation or rescheduling of appointments.


At Gateway Pediatric Dentistry, we offer patient forms online so you can complete them in the convenience of your own home or office. You can fax or email us your printed and completed forms, or bring them with you when you visit!

Emergency care

We are here for you at Gateway Pediatric Dentistry, especially during emergencies.

It is very important to be prepared in case your child has an emergency. Emergencies are inevitable, and we do our best to be available to you if one should ever arise. If your child is experiencing a true dental emergency, please call us as soon as possible and we will fit you in immediately. If our office is not open, please go to your nearest emergency center as soon as possible, or contact our on-call dentist, whose number can be found on the office answering machine.  


Here at Gateway Pediatric Dentistry, we do our best to make the dental process as seamless as possible for you. While we don’t direct bill, we do offer electronic submission to most insurance companies, so you will be reimbursed directly from them without having to deal with the paperwork.

We also understand how expensive dental procedures can be. We never want cost to defer your child from getting the treatment they need, so we will do our best to work with you to make sure cost is never an issue. As well, we will work with you to help you understand your dental coverage as best as possible.
